What it does
123FormBuilder creates online registration, payment, application and survey forms. Conditional rules show relevant fields, redirect respondents or send different emails. Its free Basic account includes five forms and 100 monthly submissions, with paid plans for larger forms, uploads and more responses.
Three kinds of rule
123FormBuilder separates conditional behavior into Field Rules, Form Rules and Email Rules. Field Rules show or hide questions, Form Rules choose a destination after submission, and Email Rules notify selected recipients when an answer meets a condition. For example, a job application can reveal different questions depending on the applicant's employment status.

Choose Get started under Basic to create a free account without a card. Basic allows 10 fields per form and includes conditional logic, but not uploads or formula fields. Gold removes branding and adds those fields. A custom form domain starts on Platinum.
