What it does

Checkbox Survey creates questionnaires with conditional questions, page branching and reporting. Rules can use earlier answers, linked contact details or properties of the response. Choose a hosted subscription or a quoted installation on your own servers. A fourteen-day cloud trial lets organizations start with an account and a chosen data-storage region.

A Checkbox condition decides whether an item or page appears; a branch moves the respondent to another page or the end. On the page being left, select its logic icon and Page Branching, then choose the destination and triggering conditions. Skipping directly to a particular question combines page branching with item conditions.

For an answer-based follow-up, edit the item, expand Conditions and select Question as the condition type. Contact Attribute and Response Property are other sources. Contact rules require the response to be linked through a Checkbox email invitation or respondent login. A general anonymous link does not supply that relationship.

Plan Annual response allowance Included users
Solo 120,000 1
Team 300,000 5
Business 1,000,000 5; extras available
Enterprise Unlimited Custom
On-Premises Unlimited Custom

Cloud plans use recurring subscriptions. Business adds the REST API, webhooks and custom retention policies; Enterprise and On-Premises require a quote. The latter runs on your own servers.

The 14-day cloud trial needs no card. Register with an email, password and company details, choosing data storage in the US, Canada, Australia or Europe.

  • Page branches and item conditions provide different controls over the route

Watch out for

  • Contact-based conditions require an email-invitation or login link to the contact record

Best for

  • Survey teams routing known respondents using answers and stored contact context
