What it does
eForm, formerly FSQM Pro, builds WordPress forms for surveys, quizzes, payments and feedback. Conditional questionnaires can number only the pages currently visible. The plugin is purchased separately from optional reporting add-ons.
If conditional logic hides pages, fixed numbers in titles can leave gaps. In Tab/Page settings, use %%NUM%% in the page title. Numbering starts at one and adjusts as pages appear or disappear, keeping a branching questionnaire's visible steps in order.
Install the downloaded plugin through WordPress admin, then use eForm → New Form. To activate automatic updates, retrieve the purchase code from your marketplace account's Downloads area, enter it under eForm → Settings and save.
Purchase and reporting
The Regular licence is USD 39 before tax and handling fees. It covers a single end product whose users are not charged, with future updates and six months of vendor support. A support extension is optional.
Exporter for eForm is a separate paid add-on for reports in formats such as XLSX, PDF and HTML, plus raw submission exports. Check that add-on when a spreadsheet or report format is essential to the workflow.
Strengths
- Visible steps renumber when conditional pages change
- A separate Exporter expands report and submission formats
Watch out for
- Extended export formats require an additional paid product
Best for
- WordPress site owners building questionnaires with different page paths
