What it does

Formsite builds and hosts online forms with conditional pages, repeating sections, response notifications and exports. Its free account allows five forms, 10 results per form and 50 MB of file space. New accounts start with a 14-day Professional trial before reverting to the free level.

Collect several entries in one response

Formsite distinguishes Item Rules from Page Rules. Item Rules show or hide an individual field, while Page Rules change the route through the form. A repeating page can collect a variable number of entries in one result, such as several attendees on the same registration.

Add a Page Break from Formatting Items to divide the form, then set Page Rules on that break. A route can lead to a later page or a success page. Skipped items are overwritten with blank answers, so page skipping should not be used as a shortcut for navigating a completed form.

Trial, free account and larger forms

The 14-day Professional trial needs no credit card and automatically becomes Free afterward. Free includes five forms, 10 stored results per form and 50 MB of file space. Personal increases those limits to 500 results per form and 500 MB. Professional allows 10 forms and 1,000 results each, with Formsite branding removal.

Business adds a sub-user, PDF results and workflows linking multiple forms. Custom PDF or Word result documents use a separately priced add-on. Account upgrades are under User Menu > Billing > Upgrade/Renew.
