What it does
Formstack Forms is a paid online form builder for business requests, applications and other data collection. Prefilled links, conditional logic and approvals help connect a form to an existing process. Try the Forms service for 14 days without entering card details; a paid account is needed afterward.
Prefill, collect and route a request
Formstack can send a form with known details already filled in, apply conditional logic and route the response through approvals. Connections include Salesforce, HubSpot, Stripe and Airtable. This is useful for organizations collecting requests that need to reach an existing business system.
Forms or a larger suite?
| Option | Included allowance |
|---|---|
| Forms | One builder user, 25 forms, 1,000 submissions per form and 2 GB storage |
| Suite | Three builder users, 100 forms, 5,000 submissions per form, document generation and e-signatures |
| Salesforce or Enterprise solutions | Sales-assisted plans with separate capabilities and limits |
Use Try It Free to create a trial account and build forms. Formstack's core Forms evaluation runs for two weeks and does not request a card. The Forms subscription does not include the entire Documents and Sign suite, so choose the bundle if generated documents and signatures are part of the same job.

Watch out for
- Continued use after the 14-day trial requires payment
