What it does
HeyForm creates conversational forms, surveys, quizzes and polls. Projects group related forms inside a team workspace, and the editor supports conditional paths and question limits. AGPL-licensed code is available for self-hosting.
Create an account, name a workspace and add a project for the forms you want to keep together. Inside the project, begin with a template, a blank form or assisted generation. Drag questions to reorder them; the question menu also offers Duplicate.
The Options Panel controls required answers, character or choice limits, images and conditional paths. Changes save automatically as a draft. Click Publish at the top right when the form should become public, then share it. This separation lets you prepare a questionnaire without making each edit immediately available to respondents.

Annual prices exclude tax. Plans include unlimited forms. Basic and Premium cap uploads at 10 MB and charge payment commissions of 3% and 1%; Business and Enterprise have no platform commission.
Managed access starts with a seven-day trial. Self-hosting the AGPL v3 code is a separate option and requires operating the application and its dependencies; the Docker guide covers the database, file storage and installation settings.
