What it does
MachForm creates web forms, surveys and approval-based submissions. The same product is available as a managed Cloud service or a self-hosted application for a PHP and MySQL server.
Log in to the MachForm admin panel and choose Create New Form. Click a field type to append it, or drag it to a chosen position. Select a field to change its label and properties, then use Save Form. Back in Form Manager, View opens the result and Code supplies the website embed.
For an existing form, expand its row and choose Edit. Field Properties changes one question; Form Properties controls the overall confirmation message, redirect and schedule. Entries opens the response manager, where Select Fields changes visible columns and Filter Entries narrows the results. Export downloads selected or all entries as Excel, CSV or tab-delimited data.
Cloud or your own server?
Cloud includes hosting, updates and backups. Choose a US or EU data centre during signup. Its plans allow unlimited forms but differ in collected entries and file capacity.

Cloud is billed monthly or annually in advance and is non-refundable. Self-Hosted is a one-time licence for a PHP and MySQL server that you maintain, including its updates and backups.

Each self-hosted licence allows unlimited forms and entries and includes one year of support and upgrades. Renewal is optional: without it, the purchased version keeps working, but further updates and support stop. A self-hosted administrator signs in on their own website; Cloud users sign in at their chosen forms-db.com subdomain.
